Enhanced Data Sharing Module
EDSM represents the process allowing the sharing of your medical records with persons involved in the provision of your care, which might be with other services outside the GP practice where you are registered.
Patients can consent to sharing their records with other parts of the NHS , such as District or Community Nurses, Children’s services, palliative care and social services, as well as Walk-in centres, hospitals. If you wish to opt in or out of this process, then you can obtain a form from Reception. Where patients have not expressed their preference for EDSM, we share your medical records, ONLY with other services treating you. If you do not want your record shared with other organisations treating you then please let reception know. Full details of this are explained in the leaflet on this page.
This practice complies with other directives and shares the records of children under the age of 13 years or under the care of the Macmillan Nursing team, for child protection reasons. This enables any decisions made about care to be accessed as appropriate, so the patient or relatives/carers do not have to be asked distressing questions possibly on multiple occasions.
