Before filling out a registration form, check you are in our catchment area.

Registering with the surgery

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

To register with the surgery:

We request that you provide two forms of identification.

These need to be:

  • A confirmation of your address.
  • A form of photo identification.

You may be contacted following your registration to arrange a new patient check appointment with one of our healthcare assistants depending on whether you are on any medications or suffer from any health problems.

As it often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.

Temporary residents

If a person is staying with you for a short period of time and becomes ill, they can obtain medical care from the practice. We ask that you complete a temporary resident form. If you go on holiday in the United Kingdom and become ill whilst away from home, you can also register as a temporary resident at a GP practice near your holiday venue.

Students living away from home whilst attending college/university should register with a surgery in the area where they spend the most time.
